(All Junior exhibitors MUST complete this form!)

Junior Expectation Form
(All Junior exhibitors MUST complete this form!)

Junior Health Form
(all Junior Exhibitors staying in the Tommy G. Thompson Youth Center
must complete this form)

 
Chaperones must obtain their registration form, expectation form
and health form from their local County Extension Office.
